What is Person-centered Planning?

Person-Centered Planning (PCP) is a set of planning approaches designed to assist someone to build a positive life and supports … used most often as a life planning model to increase an individual’s personal self-determination and independence. Read more.

What is a PATH?

The PATH is a person-centered planning tool to help an individual identify their desired future and then work backwards into creating action steps to help them achieve this future.

What is the role of the facilitator?

The role of the Facilitator is to honor the Focus Individual’s interests, dreams and culture, while including their Support Circle and keeping the planning process moving according to the identified planning steps.

Can a person-centered planning process be facilitated by one person?

No, all person-centered tools and processes are designed to be managed by a Facilitator and Recorder.

What is the NOW?

The “Now” is a snapshot of your current situation including where you live, how you feel about your life’s circumstances, who your friends are, whether you work or don’t work, volunteer, where you spend your leisure time. 

Is Person-Centered Planning required by Colorado Medicaid for consumers receiving long term supports and services?

Yes, person-centered planning was legally mandatted as part of Medicade for all consumers receiving long term services and supports through Medicaid.

What is a circle of Support?

A Circle of Support is a group of people that comprise a PATHfinder’s personal network.  It includes family and close friends, acquaintances and neighbors, school and work contacts, service providers and contacts the PATHfinder would like to connect to. 

What is a PATH Seeker?

A PATH Seeker is the individual seeking a better future and who is the center of the person-centered planning process.

What is the role of the Recorder?

The role of the Recorder is to capture the essence of the planning process in symbols and words, as a visual reminder to all participants of planning ideas, action steps and what is most important to the Focus person or PATH Seeker.

What is Graphic recording:

Graphic recording is the process of visually capturing the thoughts, ideas, commitments and actions determined during the PATH process.

What do you mean by “the dream?”

Dreams are your greatest hopes in life and are an expression of your identity, strengths, beliefs and talents.  Dreams can be pie in the sky; they guide and inspire us to keep on the path of reaching for what we want in life.

Does a PATH meet the requirements for Person-Centered Planning required by Colorado Medicaid?
